The topic this month is “Post-Hospitalization Care.” Join our Emerson Health panelists as they provide information on how to skillfully navigate life changes that aging might bring. If you or someone you care about was recently hospitalized, we encourage you to attend our virtual discussion. Thursday, September 28 at 7-8PM via Zoom Topics for discussion include selecting a secondary healthcare setting versus aging in place and recovering at home with the assistance of rehabilitative services, managing and/or mishandling medication and available support
